I understand that Vacation Bible School activities may include inflatables, bounce houses, water games, outdoor recreation, sports, and other age-appropriate activities. I acknowledge that participation in these activities involves inherent risks, including but not limited to slips, falls, collisions, weather-related incidents, and other injuries that may occur during normal participation.
I certify that my child is physically able to participate in these activities and that I have disclosed any known medical conditions, allergies, or special needs that may affect participation.
In consideration of my child's participation in Vacation Bible School, I voluntarily assume all reasonable risks associated with participation and release, waive, and discharge Bardstown Church of God, its pastors, staff members, volunteers, ministry leaders, and representatives from liability for injuries, illnesses, losses, or damages that may occur as a result of participation, except in cases of gross negligence or willful misconduct.
I understand that every reasonable effort will be made to provide a safe environment for all participants.
